Below is an easy-to-follow, four-step guide outlining how we assess and initiate home care services, from the initial intake interview to the start of caregiving.
During this interview, we will gather important details about your loved one’s care needs, discuss eligibility requirements, and answer any questions you may have about the process.
Due to ongoing office construction, we are now conducting intake interviews in the comfort of the client's home.

Submitting Documents to Case Management
Once the intake interview is completed, you will need to submit the required documents to our case management team. These documents typically include medical records, proof of Medicaid eligibility, and any relevant forms related to your loved one’s care needs.

Nurse Assessment Scheduling
After the documents are submitted, our case management team will schedule a nurse assessment. This assessment will take place at the client’s home, where a licensed nurse will evaluate the level of care needed and determine how many hours of caregiving services will be provided.

Approval from Georgia Department of Community Health (DCH)
Following the nurse’s assessment, the case report is sent to the Georgia Department of Community Health (DCH) for final approval. The approval process typically takes 30 to 45 days. Once approved, you will be notified, and care services can begin.
